Medical Transcription
& Coding:
MEDICAL TRANSCRIPTION:
Medical transcription is accurate and swift conversion of medical records
dictated by doctors or other medical professionals into electronic form. This
includes - patient history, reports, clinical notes, office notes, operation
reports, medical recommendations, letters, psychiatric relations, laboratory
reports, etc. It begins with receiving the dictation by tape or digitized
format, which is converted into a word/ text document. Nowadays, the client
leaves his dictation in a toll free number that is converted into a desirable
audio format and stored onto a dedicated server. The transcription unit logs
in to that server and downloads the dictation material. It is then converted
into word/text document using earphones and keyboards. The specialized word-processing
packages, which are used for medical transcription, include a medical dictionary
so that the person converting the voice files does not face any problems.
The transcription unit has to promise low down time and extremely high reliability/
accuracy of transcription. This is mission critical process allowing for very
thin margins for errors.
This may be carried out for any medical professional operating out of a small
clinic or a large hospital. The type of records include History & Physical
Reports, Clinic Notes, Office Visits, Operative Reports, Consultations notes
, Discharge Summaries, Letters, Psychiatric Evaluations, Laboratory/X-ray/
Pathology Reports and other similar kinds of records.
These transcribed records are used for archiving patient records, references
or for serving as a legal proof of medical advice. This record is used to
file claims from the various insurance companies. This record also serves
as a first time defense for the doctors who are sometimes sued by the patient's
for performing procedure by mistake.
In our MT division, we have nearly 100 trained professionals who can be categorized
as medical transcriptionists, Editors, QA's & Supervisors. Currently we
have a capacity of transcribing 33,000 lines per day. We have had transcription
exposure to live files in different fields.

QUALITY EVALUATION
AT OUTSOURCING INDIA
Various levels of quality checks are being adopted after the file is transcribed.
The MTs transcribe the files and send them to Editors who edit each file.
A ratio of 1:3 is being adopted i.e. 1 Editor for 3 MT's. The minimum accuracy
level to be achieved by the editor is 98%. After editing and checking, the
editor sends the file to QA for quality check to ensure that the minimum accuracy
levels are met. A ratio of 1:3 is adopted i.e. one QA for 3 editors. The minimum
accuracy level to be achieved by the QA is 99%. The file is later reviewed
by proofreaders and eliminate any discrepancies that might have occurred while
formatting demographics and MT ids etc. Ones the files are ready, it is being
uploaded by the software engineer. Overall supervision from downloading of
files quality of transcription, turn around time compliance is done under
strict supervision of Production Manager. We operate 24 hours a day 7 days
week and 365 days in a year.
We have been effectively transcribing all reports/summaries such as Emergency
Room, Reports/Discharge Summaries/ Pediatric Reports/ Surgical Procedures,
History and Physical Report, Operative Report, Chart Note and Pathology Report.
A response time of between 8-24 hours and an accuracy of over 98% is the accepted
norm in Medical transcription.
